Hello Nampa PD employees, 

The III-A is happy to host an onsite Wellness Screening & Flu Shot Clinic on Wednesday, September 15th from 1pm-5pm at Nampa PD: Baker Room – at no cost to the member.

Additional details:

• If you participated in the spring screening (May 2022), you do not need to sign-up again for another screening, but are eligible for a flu vaccine.
• Employees & spouses on the III-A medical plan may participate and dependents age 12 and over may receive a flu vaccine. *If spouses and dependents are participating, they can share a time slot with the employee.
• All results, along with educational material, will be mailed to the
  Agency (in sealed envelopes) to be distributed within 2 weeks.
• Those with concerning elevated results will receive a follow-up call
  from the Nurse Practitioner.
